Community Programs – Clinical Services

DOMAIN

Linking successful community programs and clinical services increases access to services, and the ability for people to prevent and manage disease. Making effective community interventions more widely available and tying them to doctor referrals helps to increase the reach of the program in the community. These types of partnerships and links between hospitals and other health care providers, possibly through community benefit investments, between services can improve overall community health.

Cultivate strong connections linking individuals, community organizations, businesses, schools, the health care system and other partners to improve health outcomes, reduce health care costs, and improve quality of life.
